Brakes and clutch

Not good news I'm afraid. Sounds like you need a new clutch plate. Once the plate wears too thin, the pressure plate is unable to grip it and it slips under load. Easiest thing to do is to remove the seat base, then either completely remove the gearbox, or slide it back and replace the clutch. It is best to replace the driven plate, the pressure plate and the thrust race though you may get away with just replacing the driven plate.
As for the brakes, you don't say if you have power brakes or not. All kinds of possibilities from adjustment, linings, cylinders and even possibly the servo (if you have one).
